Creating your first form

Learn how to use the drag-and-drop form builder.

Start with a template or build from scratch

When you log into IntakeEase, you'll land on your Forms dashboard. Click the "New Form" button in the top right.

You have two options:

  • Start from a template — Choose from 5 pre-built forms designed for coaches (Coaching Agreement, Client Discovery, Payment Authorization, Practice Policies, or Cancellation Policy). Templates are fully customizable after you select them.
  • Start from scratch — Begin with a blank form and add your own fields.

Adding fields to your form

The form builder uses drag-and-drop. On the left side, you'll see the field types available:

  • Text fields — Short text (one line) or long text (paragraph)
  • Contact fields — Email and phone with built-in validation
  • Date picker — For scheduling or date-of-birth collection
  • Choice fields — Checkboxes, radio buttons, or dropdowns
  • Signature — Digital signature capture that works on any device
  • Display elements — Section headers and paragraph text for instructions

Click any field type to add it to your form. Then click on the field to edit its label, help text, and whether it's required.

Previewing and saving

Use the Preview button to see exactly what your clients will see. The preview works on desktop and simulates mobile view.

Your form auto-saves as you work, so you won't lose progress if you close your browser. When you're ready, click Save to finalize.

Tips for effective intake forms

  • Keep it under 15 fields when possible — completion rates drop after that
  • Put the signature field at the end
  • Use section headers to break up long forms
  • Write clear, simple labels (avoid jargon)
